What is Obsessive-Compulsive Disorder

OCD is a very challenging type of mental health problem which typically involves intrusive thoughts (called ‘obsessions’), which pop into your mind involuntarily and often challenge your very sense of self. Intrusive thoughts may have particular themes, such as being about germs or contamination, danger or safety, culturally unacceptable such as about causing harm or sexual content, or about you as a person. They are distressing and often hold some meaning about you, such as ‘this means I’m a bad person’ or ‘this means I’m at risk of danger’. To manage these, you may use certain repetitive behaviours, known as ’compulsions’. These can be any behaviour – visible or otherwise – that is designed to stop the intrusive thought in some way. For example, compulsively cleaning your hands due to a fear of catching germs, or compulsively counting or repeating due to an intrusive fear of forgetting. OCD comes with significant, strong emotions, such as anxiety, disgust and guilt, and can be very impactful. Excessive reassurance-seeking is another sign of OCD.

What OCD Therapy Involves

Our OCD specialist psychologists use the evidence-based, frontline treatment for OCD, which is called Exposure and Response Prevention (ERP) and cognitive behavioural therapy (CBT). OCD therapy basically involves gradually, systematically exposing yourself to the feared outcome in order to realise that OCD feeds you mistruths. It also involves trying to separate your intrusions from yourself through addressing patterns of thinking, beliefs and responses to the intrusions.


Approaches used in OCD therapy are:

  • Identifying what types of intrusive thoughts you have and what you think they mean (e.g. that they are likely, mean something about you and that you are responsible for them)

  • Addressing and challenging the thinking and beliefs you hold about your intrusions and compulsions

  • Thought experiments to rebalance any attempts to control or predict thoughts

  • Creating a personalised exposure hierarchy and working through this, often with the therapist’s support in session

  • Reducing avoidance of anxiety and underlying emotions to allow your brain to learn that they need not be feared and avoided

  • Gradually limiting your response to your intrusions and anxieties

What types of OCD can your specialist psychologists treat?

Our OCD specialist psychologists treat all presentations of OCD, including contamination and germ OCD, harm obsessions, taboo or intrusive thoughts (religious, sexual, paedophilia OCD), symmetry and ordering, relationship OCD (ROCD), health OCD, Pure O (predominantly obsessional OCD), and OCD with excessive reassurance-seeking. We also work with Body Dysmorphic Disorder (BDD) and health anxiety, which share similarities with OCD in how they work.

What is Exposure and Response Prevention (ERP), and is it the most effective OCD treatment?

ERP is the gold standard, NICE-recommended psychological treatment for OCD and is significantly more effective than general CBT for this condition. It involves systematically and gradually exposing yourself to the situations or thoughts that trigger your obsessions, while resisting the urge to carry out compulsions. This teaches your brain that the feared outcome doesn't occur and that anxiety naturally reduces without the compulsion — breaking the OCD cycle. Our therapists guide you through this process carefully and supportively.

My intrusive thoughts are very disturbing — will my therapist judge me for them?

Absolutely not. Intrusive thoughts — however distressing or seemingly shameful their content — are recognised as a core feature of OCD, not a reflection of who you are as a person. Our OCD specialists understand this deeply and create a non-judgmental, matter-of-fact space to work through even the most difficult thought content. Many clients feel significant relief when they realise their therapist is completely unfazed by what they're experiencing.

I've tried CBT before and it didn't help my OCD — why would this be different?

Standard CBT delivered by a non-specialist is often ineffective for OCD — or can even make it worse if the wrong techniques are applied. OCD requires specific, specialist treatment using ERP delivered by a therapist with dedicated OCD training and experience. Our psychologists are OCD specialists who understand the nuanced and counterintuitive nature of effective OCD treatment. If you've had unhelpful therapy before, this does not mean therapy can't work — it often means the right approach hasn't yet been applied.

Can OCD therapy be done online or does it need to be in person?

OCD therapy, including ERP, can be delivered very effectively online. Research supports online delivery of ERP with equivalent outcomes to in-person treatment. In fact, for some clients, working online has specific advantages — for example, being able to practice exposure exercises in their own home environment where OCD triggers are most present. We offer both online and in-person sessions across Warwickshire and Birmingham.
