Cancellation Policy
Your appointment has been booked - please find the Zoom link in this email and a diary holder attached. Please make sure you complete all of the forms below before your appointment, which includes all of the questionnaires. If the forms are not completed before the appointment, we may need to re-arrange your appointment. Please note we reserve the right to cancel sessions where the invoice has not been paid by the due date.
