Cancellation Policy
Your appointment has been booked - please find the Zoom link in this email and a diary holder attached. Please make sure you complete all of the forms below before your appointment, which includes the terms and conditions and the questionnaires. If the forms are not completed before the appointment, we may need to re-arrange your appointment.
